Transitioning Patients from Curative to Palliative Care

Sometimes when a patient transitions from skilled home health care to hospice care, other agencies introduce a new team of health care providers causing undue stress on the patient and his or her family. Patients enrolled in our home health care who transition to end-of-life care may expect the same team of caring health professionals to provide treatment.
